Background
The CancerHelp Institute offers a grant for a touchscreen monitor to be used with the CancerHelp cancer patient education software at hospitals, cancer centers, oncology practice offices, wellness centers and libraries. The CancerHelp patient education software is part of an ongoing study to evaluate the effectiveness of providing National Cancer Institute (NCI) information and other information to patients with a touchscreen computer system. Currently more than 200 subscribers utilize the CancerHelp patient education software in their facilities.
Touchscreen Monitor Grant
The CancerHelp patient education grant program provides a new 17-inch flat panel touchscreen monitor to be used with the CancerHelp patient education software. The touchscreen is valued at $650. As a CancerHelp subscriber, your facility would provide a Windows compatible computer and printer (view or print detailed specification sheet, Computer Requirements). Each subscriber will receive monthly CD-ROM updates to CancerHelp (twelve annually). Once per quarter the CD-ROM monthly mailing will include a disk to collect user statistics for return to the CancerHelp Institute. Each subscriber will receive quarterly and end-of-year reports of their user statistics.

Subscription Cost
Grant participants must subscribe to the CancerHelp software for $3,000 each year for the two year duration of the grant and pay a one time $250 monitor shipping and handling charge. The total financial commitment over two years equals $6,250; Year I = $3,250, Year II = $3,000. This covers the cost of providing the monthly CD-ROM updates to the software, the processing of user statistics, and technical and marketing support.

Optional Kiosk Cabinet with Flat Panel Touchscreen Monitor
For an extra fee of $3,680, in addition to the $3,250 first year subscription and shipping fees, you can order a custom kiosk cabinet with Flat Panel Touchscreen monitor for your facility. The kiosk cabinet is manufactured by Olea Exhibits and is customized to run the CancerHelp patient education software. The $3,680 is payable to Olea Exhibits and includes the kiosk cabinet, the flat panel touchscreen monitor, the installation of your computer and printer and the shipping of the kiosk to you.
The benefits of a kiosk cabinet are the following:
Extra security. The cables and computer components are locked up inside the cabinet.
Marketing. The kiosk cabinet helps to clearly identify the CancerHelp system as an education resource for patients and family members.
Customization. In addition to custom screens in the screen saver and your name and logo on printouts, the kiosk allows you to customize the lighted sign at the top of the kiosk. The sign can display your name and logo.
The Kiosk cabinet that we recommend for use with CancerHelp is manufactured by Olea Exhibits. The Manhatten kiosk model has been tested with CancerHelp and has a slot and built in paper output tray for printouts from the CancerHelp software.

There are several additional requirements if you select the kiosk option:
1) Your facility will need to supply a Windows XP compatible computer and laser printer that meet certain specifications. The computer will need to be a horizontal or desktop model and not a tower model.
2) You will need to ship the computer and printer to the CancerHelp Institute for configuration and testing. The CancerHelp Institute will then send the computer and printer to Olea for them to install in the kiosk and test with the touchscreen.
3) The CancerHelp Institute will not provide a grant for a touchscreen if you decide to choose the optional kiosk cabinet. However, the CancerHelp Institute will not charge any additional fees for the following work:
Configuration and testing of your computer and printer with the CancerHelp software including the Web access feature if you have chosen this option.
Coordination with Olea Exhibits to properly install the computer, printer and touchscreen for shipment to you.
Please call the CancerHelp Institute toll free at (877) 280-4790 when you are in the planning stages of acquiring a kiosk cabinet for your CancerHelp computer touchscreen system. There may be updates to the kiosk pricing, preparation steps or changes to the equipment specification. Please allow extra time for the kiosk coordination.

Selection Criteria
The grant recipients will be chosen from a diverse cross section of facilities around the country including, but not limited to, those serving underserved rural and urban populations. Both established and new cancer patient education programs should apply for this grant. Of special interest are programs that would maximize the use of CancerHelpfor example, by locating CancerHelp in a patient waiting area.

Application Instructions
Each applicant must complete the Grant Application Form. The Grant Application Form must be mailed, faxed or e-mailed by October 22, 2007.
Two continuation pages may be used to complete the Grant Application Form. The Grant Application Form must include answers to the following questions:
How many patients does your cancer treatment program serve annually? List inpatient and outpatient numbers separately, if available.
Briefly describe the demographic (i.e. age, race) breakdown of your patient population annually. List inpatient and outpatient numbers separately, if available.
If you have an existing education program, what information is given to patients and how is it delivered?
How has your facility demonstrated a commitment to cancer patient education?
How do you plan to use CancerHelp to support your cancer patient education program?
What are your plans to publicize the availability of CancerHelp at your facility?
Please submit printed materials that describe both your cancer treatment program and your existing patient education program, if available. These materials should be submitted to the CancerHelp Institute along with your application by October 22, 2007.

Notification
Grant recipients will be notified of their selection for the CancerHelp cancer patient education grant within 1-2 weeks after submitting the grant application. Grant recipients will receive an invoice for the first years $3,000 annual subscription fee and the $250 monitor shipping and handling fee at the time they are notified of their selection for the grant. The $3,000 annual subscription fee and the $250 monitor shipping and handling charge are due within 30 days of notification of acceptance. The touchscreen monitor and CancerHelp software will be shipped within 2-3 weeks after receipt of the first years payment ($3,250).

Ongoing Operation
At the end of the two year grant, each participant has the option to pay the $3,000 annual fee and renew the subscription to the CancerHelp software. The CancerHelp Institute will continue to provide monthly updates to the software and quarterly user statistics reports. At the end of the two year grant period, the touchscreen monitor may be used by participants as long as they continue to subscribe to the CancerHelp software. If the subscription to CancerHelp is not renewed, the touchscreen monitor must be returned.
